You may download the applications necessary to begin the application
process for the Ely Center, a center that focuses on social communication
skills and social pragmatics for children and young adults.
Please download and complete all the forms in the packet.
Once completed, please return them to the office. When we receive
the paperwork, we will contact you to schedule an intake interview.
The purpose of the interview is for group placement only. We ask
that any related information or materials be forwarded to us prior
to the interview as it aids us in the placement process. At the
Ely Center, our goal is to create homogenious groups where all the
clients feel safe and comfortable. There is a $50.00 administrative
fee. Please make the check payable to The Ely Center.
